The claims process uses a lot of legal terms and lots of words. By design, it is very confusing. All the mailings after acceptance are way too complicated. Again - designed for the lawyers, not the users. That is why we hired Atomic Workers. Hire an advocate. Hire them and then let them do the work. Spend your time as a caregiver, not a paper pusher.
